Form AF 1206, Nomination for Award, is a U.S. Air Force document that puts an individual award package in front of the rating chain and the senior leaders who pick award winners. Squadrons and supervisors use it to nominate active-duty Air Force and Space Force members, civilians, or units for quarterly, annual, and special recognition awards. The single page captures duty information and a structured narrative of the nominee's accomplishments during the rating period.
AF 1206 turns a supervisor's recommendation into an Air Force award package decision-makers can compare side by side. Boards rely on the form to:
Enter the nominee's name, rank, organization, and award category at the top.
Confirm the inclusive dates of the rating period.
Write three accomplishment groups, each with bullets covering scope, action, and result.
Quantify every bullet with metrics, savings, awards, or measurable results.
Verify classification markings, length limits, and AFI 36-2805 formatting.
Review the package with the rater before sharing the completed form.
Air Force and Space Force supervisors and commanders complete AF 1206 whenever they nominate an Airman, Guardian, civilian, or unit for a quarterly, annual, or special recognition award. The supervisor closest to the nominee usually drafts the package, and squadron leadership endorses it before forwarding to the awards board.
After the form is signed and routed, the awards board uses it to compare nominees, score the accomplishments, and select winners at the squadron, group, wing, and major command level. Approved packages drive announcements, certificates, and entries in the member's record.
Members who are not being nominated for a specific award do not need an AF 1206. Decorations such as the Air Force Achievement Medal or Commendation Medal use AF Form 642 instead, and joint service awards follow DoD-wide forms tied to the joint command. Routine performance feedback uses the airman comprehensive assessment process and stays separate from this nomination package.
The deadline for AF 1206 follows the suspense set by the awarding command, which is usually two to four weeks before the awards board convenes. Quarterly packages line up with the start of each new quarter, while annual packages line up with end-of-year boards. Late nominations are typically returned without action, since boards score nominees on a fixed timeline.
The U.S. Air Force issues AF 1206 through the Air Force e-Publishing forms library on its public site, and many wings also keep a current copy on internal sites. Prefer to draft the narrative before you print? Open the blank in our editor, fill it on screen, and download a clean copy — routing it up the chain happens outside our platform.
AF 1206 must be signed by hand in ink, since the Air Force routes the package up the chain with wet signatures from the rater, the additional rater, and the unit commander. After you fill the form on our platform, download the completed file, print it, and pass it for ink signatures and dates. Checking the latest AFI 36-2805 guidance is a good idea before finalizing.
Once your AF 1206 has the rater, additional rater, and commander signatures, the squadron forwards the package up the chain through the group, wing, and major command awards programs. Most units send it through their official email box, the awards SharePoint site, or interoffice mail to the awards monitor at the next level.
